Once you have an account at Bethpage, you may experience some life changes that will require you to make some changes to your account. Download, fill out and mail or fax in the forms you need:

In order to open a Trust Account with Bethpage, the forms and documents below are required. Download the required forms, fill it out and bring it in to your local Bethpage branch.

• Copy of valid photo identification
• Trust document or Certificate of Trust – including Successor Trustee(s)


• Signature Card – completed and signed by all Trustee(s)
• Trustee Affidavit – completed and signed by all Trustee(s) and notarized
• Fiduciary Account Documentation Requirements

In order to establish a Power of Attorney with Bethpage, the forms and documents below are required. Download the required forms, fill it out and bring it in to your local Bethpage branch.
